Did you receive a fax-back request? On some accounts we require that customers fax back the following form, as a little bit of security against fraud (something we fall victim to pretty often, unfortunately).
If you didn't receive the fax back notification or you sent it and were never approved, go ahead and email me directly at email@example.com and I'll take a look at it for you.
