Formail configuration not working, please help

Hello all,
I recently had someone work up some web pages for a website I have, unfortunately they did not finish the web email form, I have been using the following to guide me *he with little knowledge), but so far I cannot get it to work:

This is the code:

Obviously bookings@domain.com is replaced with the real domain name, that email is correctly set up in my web panel which forwards onto my regular email, I have sent a test email to it and it correctly forwards it, so I believe there is something wrong with the form, please help!!

The first form action is deleted, I left that in to show what the original was like.

OK I think I got it working somewhat, here is the new code, I added a submit success page, not sure what else I did correctly:

I think thats great, only think would be nice to change is:

WWW Form Submission‏ - is the title of the email, any idea how I may change this?[hr]
and that was easy to figure out, woo!
